Welcome to the West Mendip PrimarY Care Network

Primary Care Networks (PCNs) have been set up around the country as part of the changing landscape of your NHS

The West Mendip PCN formed on 1st July 2019

It represents the collaborative efforts of 5 GP practices to
 develop proactive, personalised, integrated health and social care

WHO Are we?

West Mendip PCN is one of 13 PCNs in Somerset. The map above shows the entire Somerset PCN map.

Our PCN is made up of 5 GP surgeries working together, and you can access each of our websites by clicking on a name below:

What is a PCN?

​NHS England have created a helpful animation that explains the concept of PCNs & how this new way of working enables health & other services to work together to provide better access and care for patients.

WHAT are our aims?

In order to provide a proactive approach to supporting our population's health and care, we have seven areas we will focus on:

Five will start this year:
  1. Structured medication reviews
  2. Enhanced health in care homes
  3. Anticipatory care ​
  4. Personalised care
  5. Support early cancer diagnosis

​The remaining two will start by 2021:
  1. Cardiovascular disease 
  2. ​Tackle inequalities​

Staff Newsletter

The PCN will produce an editorial-style quarterly staff newsletter, also available for patients to view

third party organisations

As the PCN grows, it will naturally be looking to network with providers outside the traditional primary care model, including third sector services

To allow a central repository of such requests, and to ensure the request is appropriate for PCN engagment - please contact the Somerset Local Medical Committee (LMC) in the first instance who will then distribute information where suitable
